Phyllanthus niruri Linn increase infiltrating lymphocyte and apoptosis of rectosigmoid cancer patients. A phase II clinical trial

Abstract

Background: Phylantus niruri linn (PNL) extract  has potential cytotoxic effect  on colorectal cancer both on in vitro and in vivo study, but have never been reported in human subject. The purpose of this study is to elaborate whether PNL  extract  also has cytotoxic effect on human colorectal cancer. 

Method: A one group  pre-test and post test design was conducted on 15 patients with non obstructive operable rectosigmoid cancer without neo adjuvant chemotherapy or chemo radiation. Before PNL extract   (100 mg Stimuno) treatment, all patient underwent rectoscopy biopsy. Stimono treatment was given at least for 14 days and then patients underwent rectal resection. Speciment from biopsy and rectosigmoid resection were manage by Department of Anatomical Pathology Faculty of Medicine Gajah Mada University Yogyakarta for evaluating the diagnosis, infiltrating lymphocyte, apoptosis index, perforin, granzyme-B and caspase-3 expression.  A pair t test was done to evaluate the significant increasing measured variables and linear regression was done to evaluate the role of individual variable. 

Result: Infiltrating lymphocyte, expression of perforin, granzyme-B and caspase-3 and apoptosis index  were significantly (p=0.000) improve after PNL extract treatment. After linear regression, variable which  significantly correlate with apoptosis was only infiltrating lymphocyte.

Conclusion:  PNL extract increase apoptosis index of colorectal cancer  through increasing of infiltrating lymphocyte.
